Paediatric Otolaryngology

Overview

What is Paediatric Otolaryngology?

Often referred to as a children's ENT specialty, paediatric otolaryngology is both a medical and surgical discipline. It caters to the unique and specific ear, nose and throat conditions affecting infants, children and adolescents. With a child's constant developing anatomy, they have a distinct physiology. Their communication abilities vary with age, and this requires a specialised approach.

Also known as a paediatric ENT specialist, a paediatric otolaryngologist is an expert with additional skill in managing the full spectrum of disorders in growing children. Anything from common recurrent ear infections and troublesome tonsillitis to complex airway abnormalities and congenital hearing loss all fall under their remit. With surgical intervention a large portion of this speciality, it goes beyond just physical care into understanding childhood development managing milestones in speech, learning and social growth.

Types of Paediatric ENT Conditions

ENT has broad scope of conditions fall under it. Congenital, inflammatory, infectious and neoplastic conditions are all covered under its scope. Disorders of the ears, such as recurrent acute otitis media, chronic glue ear impacting hearing and cholesteatoma are some key areas that fall under this scope. Conditions of the nose and sinuses are also common ENT issues. There are mainly issues such as chronic rhinosinusitis, allergic rhinitis and nasal obstruction from enlarged adenoids.

Forming a critical part of the specialty are throat and airway disorders like recurrent tonsillitis, obstructive sleep apnoea due to enlarged tonsils and adenoids, laryngomalacia and other airway anomalies. Neck masses and cysts, salivary gland disorders and paediatric hearing loss and voice disorders are also other vital areas that are managed. All diagnostic and management strategies are suited to the child's age and stage.

Parents should seek care for their children when they notice common signs like persistent ear infections, recurring sore throats or tonsillitis and chronic nasal congestion or sinus infections. Signs that shouldn't be ignored are hearing concerns, such as delayed speech development, frequent inattention or needing the television volume excessively high.

Noisy breathing, snoring accompanied by pauses in breathing, or significant difficulty swallowing also need to be looked into. Any lump in the neck, injury to the ears or nose or persistent hoarseness should be assessed. It's time to consult an ENT paediatric specialist essentially, if an ENT symptom is frequent, severe or impacting a child's sleep, hearing, eating or general development.

The diagnostic journey begins with a detailed history. Details are often gathered from the parent followed by a gentle, child-friendly clinical examination. The equipment we use are specialised and designed for the smaller anatomy of children. Otoscopy is standard for ear conditions and to assess middle ear function, tympanometry may be used while hearing is evaluated with age-appropriate audiological tests. To visualise the nasal passages, adenoids and vocal cords comfortably in the clinic, we perform a flexible nasendoscopy, using a very thin, soft camera, without the need for sedation.

We may employ low-radiation CT scans or MRI to image complex sinus disease, congenital abnormalities or deeper neck structures when further detail is required. To ensure the child's comfort and safety throughout, our approach is always to obtain the clearest diagnostic picture with the minimum necessary intervention.

Yes. At NMC, all paediatric surgeries are performed by paediatric anaesthetists that are specialised in administering anaesthesia safely to children. Only the most modern techniques and medications are used. Children will be also closely monitored throughout the procedure in a dedicated paediatric setting.
Not necessarily even though grommets are a highly effective treatment for persistent glue ear or recurrent infections. When medical management has not been successful and hearing is affected, they are then typically recommended only after a period of watchful waiting.
