What is Rhinology?

Rhinology is a subspecialty of Ear, Nose and Throat surgery focused specifically on the nose and sinuses. It manages the intricate anatomy of the nasal cavity, the paranasal sinuses and the complex connection between structure and function. This allows us to breathe, smell and filter the air we inhale. Playing role of both a physician and a surgeon, a rhinologist investigates why patients lose smell, the reason sinus infections keep returning and look into fragmented sleep by airway obstruction.

Types of Rhinology Conditions

The most common types of rhinology conditions are inflammatory. A significant portion of the UAE population are affected by allergic rhinitis, driven by dust and airborne irritants. Chronic sinusitis is a more serious inflammatory state and happens when the sinuses become chronically obstructed and infected, with or without polyps.

Deviated septum, turbinate hypertrophy and nasal valve collapse are structural abnormalities commonly treated under rhinology. Extending beyond infection, sinus pathology covers mucoceles, fungal balls and tumours that can arise within the sinus cavities. Loss or distortion of smell, known as olfactory disorders, are increasingly recognised as a significant quality-of-life issue. Post-viral inflammation and head trauma are some of the reasons causing them.

When nasal obstruction becomes persistent enough to affect breathing, disturb sleep and make exercise hard, a patient should see a rhinologist. A proper assessment is required for snoring that's nasal in origin.

Experiencing three or four episodes of recurring sinus infections a year, or a single episode that extends for weeks despite antibiotics needs to be investigation. This suggests an underlying anatomical or inflammatory driver. Losing smell can affect safety, appetite and can impact emotional wellbeing and should be investigated. Recurrent nosebleeds, persistent post-nasal drip and facial pressure that never quite lifts are all valid reasons to consult a rhinologist.

A detailed history looking into symptom patterns, triggers, occupational exposures and any history of facial trauma would mark the start of the diagnostic process. A nasal endoscopy follows, if needed. An endoscopy is a procedure where a thin telescope is inserted into the nasal cavity and sinus openings to magnify and reveal polyps, pus or a septal spur impinging on the turbinate. The gold standard is CT scanning, for chronic sinusitis as it shows mucosal thickening, fluid levels and sinus drainage pathways. MRI is reserved for suspected tumours only. Allergy testing, either skin prick or blood, can be transformative for patients with rhinitis.

Our approach is a step-by-step process, and surgery is not the default at NMC. We offer surgery when medical therapy has been optimised and found insufficient. We begin with intranasal steroid sprays, saline irrigation and allergen avoidance for inflammatory conditions. Cases that are more resistant will be put on short courses of oral steroids or biologic therapies. These help at targeting specific inflammatory pathways and have changed the landscape for severe polyp disease.

Endoscopic surgery is almost always chosen whenever surgery is indicated. Through functional endoscopic sinus surgery (FESS), we open natural drainage pathways, remove obstructing polyps or diseased mucosa and restore ventilation. This is done through no external incisions, no visible scars and typically entails a rapid recovery.

To address turbinate reduction and structural obstruction, we perform a septoplasty. Turbinate tissue has an important physiological function, and we preserve as much as possible while relieving obstruction. Conservation remains a major part of our approach.

Frequently Asked Questions

Most patients experience discomfort rather than severe pain. As with modern techniques, nasal packing is avoided making recovery significantly more comfortable. Most return to work within a week.
Once initial congestion subsides, breathing improvement is often noticed, typically within days. For four to six weeks, strenuous activity and contact sports should be avoided.
Surgery does not cure the underlying tendency toward inflammation. Wider drainage pathways are created, that are easier to manage medically. It does reduce the frequency and severity of infections dramatically.
