Otology

Overview

What is Otology?

Falling within Ear, Nose and Throat (ENT) surgery, otology is highly focused and exclusively dedicated to the ear helping diagnose, manage and surgically treat conditions affecting it. An otologist, as opposed to a general ENT surgeon, possesses advanced fellowship-trained expertise specifically in the complex structures of the temporal bone, hearing apparatus and balance system.

Otology treats anything from common complaints like recurrent infections and impacted wax to intricate microsurgery of the middle ear and skull base, addressing the full spectrum of ear disorders. Otologists treat disease as part of their practice but they also help preserve our hearing and balance, which are two of our most delicate senses. The ability to hear clearly and feel steady on one's feet is fundamental to communication, independence and quality of life.

When to Consult an Otologist

Many people experience minor ear issues but knowing when to seek a sub-specialist is crucial. Patients should consider visiting an ear clinic near then if experiences like persistent hearing loss that makes it difficult to follow conversations or requiring the television volume to be higher than others start interfering with daily.

Chronic tinnitus, such as regular or constant ringing, buzzing or hissing in the ear that interferes with concentration or sleep, is another clear sign. Recurrent ear infections in the form of multiple episodes in a year or an infection that simply will not clear with standard treatment, warrant expert review.

Often originating in the inner ear, balance issues and dizziness, particularly episodes of vertigo or recurrent unsteadiness need investigation. Patients should never ignore unexplained deep ear pain, pressure or persistent drainage from the ear canal. Medical emergencies also do appear in the ear region, like sudden hearing loss, a rapid drop in hearing often in one ear, which need urgent attention.

We move beyond a simple look in the ear. Our thorough and precise diagnostic process begins with a detailed discussion of the patient's symptoms, medical history and any impact on daily life.

A comprehensive physical examination using an otomicroscope follows. This provides a magnified, illuminated view of the ear canal and eardrum, allowing our specialists to detect subtle perforations, retraction pockets or signs of infection invisible to the naked eye.

We also employ a full suite of advanced audiological and vestibular testing within our clinics, including pure tone and speech audiometry to map the exact degree and type of hearing loss. Tympanometry is used to assess the health and mobility of the eardrum and middle ear with videonystagmography (VNG) evaluating the inner ear's role in balance by tracking eye movements. For intricate imaging of the bony ear structures and soft tissue, we deploy high-resolution CT and MRI scans which are essential for diagnosing acoustic neuromas, cholesteatoma or planning complex surgery.

  • Chronic Otitis Media: This manifests as a persistent infection and inflammation in the middle ear.
  • Cholesteatoma: An abnormal skin growth in the middle ear behind the eardrum. It causes bone erosion and hearing loss if not surgically removed.
  • Otosclerosis: A condition where abnormal bone remodelling in the middle ear causes the stapes bone to become fixed. This leads to progressive conductive hearing loss.
  • Hearing Loss: This can be conductive, sensorineural and mixed hearing loss in both children and adults.
  • Perforated Eardrum: A hole or tear in the tympanic membrane.
  • Meniere's Disease: Severe vertigo, tinnitus and fluctuating hearing loss caused by a disorder of the inner ear.
  • Vestibular Neuritis and Labyrinthitis: Sudden vertigo caused by inflammation of the inner ear or balance nerve.
  • Acoustic Neuroma (Vestibular Schwannoma): A benign tumour on the balance nerve.
  • Impacted Earwax (Cerumen Impaction): This is the complete blockage of the canal that causes pain, fullness and hearing loss.
  • Pinna Deformities and Ear Trauma: Congenital deformities or injuries to the outer ear.

In otology, treatment is never one-size-fits-all. Tailored to the specific condition, our approach investigates severity, lifestyle and expectations of our patients. We always begin by exploring non-surgical avenues. For instance, a patient visiting our ear care clinic with impacted wax will receive safe, micro-suction removal under direct visualisation. A patient with vertigo may benefit from vestibular rehabilitation therapy which is specialised physiotherapy for the balance system.

We work closely with audiologists to fit the most appropriate hearing aids to help patients requiring hearing rehabilitation. When surgery is the best option, our otologists are skilled in the latest microsurgical techniques. Often performed through the ear canal or a small incision behind the ear, these procedures are varied but are all delicate. We perform tympanoplasty which is the surgical repair of a perforated eardrum. Ossiculoplasty, which is the reconstruction of the tiny middle ear bones (ossicles) is also performed, to restore the chain of hearing. A precise procedure for otosclerosis, a stapedotomy, where the fixed stapes bone is replaced with a prosthetic piston is another solution. To treat cholesteatoma or severe chronic infection, we remove infected bone from the mastoid air cells behind the ear through a mastoidectomy. For patients who do not benefit from conventional hearing aids, we offer bone anchored hearing aids and cochlear implants.

Our NMC facilities across the UAE are equipped to deliver an integrated standard of care to patients searching for an ear cleaning clinic Dubai residents trust, or require complex reconstructive middle ear surgery.

ENT surgeons trained in otology and neuro-otology run our otology departments. They all come with focused expertise that allows them to manage the full complexity of ear disease, from routine ear clinic visits to intricate skull base procedures.

Supporting them is a dedicated network of audiologists, vestibular physiotherapists and speech therapists. We understand that discussing hearing loss or dizziness can be worrying and our consultants take time to explain diagnoses using clear, visual aids and ensure every question is answered. Surgical precision and technicalities coupled with empathy guide our consultants who can help those looking for the best clinic for ear issues specifically. Our specialists are recognised across the NMC network for providing this focused, patient-first care in Dubai, Abu Dhabi and beyond.

Frequently Asked Questions

Broader, a general ENT specialist manages conditions of the ear, nose and throat. An otologist is an ENT surgeon who has completed an additional 1-2 years of accredited fellowship. Their training is solely focused on the ear and related skull base structures. They are the sub-specialists for complex ear surgery, hearing restoration implants and chronic balance disorders.
Absolutely. The inner ear houses the vestibular system which is our primary organ of balance. Most conditions that cause vertigo originate in the inner ear like Vestibular Neuritis and Meniere’s Disease. Specific diagnostic tests are performed by otologists to determine if the ear is the source of your imbalance.
Yes. Widely considered the gold standard, micro-suction uses a gentle vacuum under microscope, allowing the otologist to see exactly what they are removing. Compared to traditional water syringing, it’s safer, more comfortable and carries a lower risk of infection or eardrum perforation.
